Re-Adopt Disconnected Unifi AP

In the Unifi GUI forget the AP in question

SSH to device

syswrapper.sh restore-default

The AP will be rebooted and the SSH session will disconnect. Once it comes back up reconnect via SSH.

set-inform http://ip-of-controller:8080/inform

Go back to the GUI and complete the adoption process
